Job Summary
The First Link Intake Coordinator is responsible for connecting newly diagnosed people living with dementia and their families to the Alzheimer Society so that they can get the help they need as soon as possible. The role of the First Link Intake Coordinator is to act as the first point of contact for clients who are referred into the Society, initiating the assessment process by collecting basic demographic information and booking appointments for intake assessments. The role also requires the First Link Intake Coordinator is to promote awareness and to strengthen community partnerships and referral systems by engaging with Primary Care Partners to facilitate direct referrals into the Alzheimer Society.
Essential Duties and Responsibilities
• Establish contacts within Primary Care and other community partners to help facilitate
direct referrals to the society
• Develop and maintain a system for triaging clients for intake assessment
• Make initial calls to clients to gather basic demographic information and set up a
client profile for further intake by First link Care Navigator
• Book appointments for clients with a First link Care Navigator for further assessment
• Ensure that client files within the client database contain all required
client demographic information
• Complete client intake process including receiving fax and web-based referrals
and reviewing relevant referral information
• Oversee internal and external referral processes
• Monitor referral sources; identify and address gaps
• Maintain and submit statistics as required
• Work in conjunction with other Society staff
• Perform other duties consistent with the job classification, as required
• Provides follow up letters to referral partners as per agency standards indicating
the status of their referral
• Enter statistical & client data into electronic client database as per organization standards
• Administer cognitive assessments using standardized tools and provide client and care partner support as required.
Hours of Work
The First Link Intake Coordinator generally completes all duties within normal business hours; however, flexibility in hours is required, with occasional evening and weekend work.
Job Qualifications
Education:
Minimum Bachelor degree in social work, gerontology or other related health care discipline. Registered health professional designation.
Experience
• Experience in office administration is considered an asset
• Experience in the health care and/or social service sectors is considered an asset
Other knowledge, skills, Abilities or Certifications:
• Excellent verbal and written communication skills in English
• Database experience an asset
• High standard of ethics and confidentiality
• Highly organized
• Knowledge of Alzheimer’s disease and other dementias
• Excellent communication skills (verbal and written)
• Exceptional interpersonal skills, including shared decision-making and facilitation
• Ability to prioritize workload and manage competing tasks
• Ability to take initiative and be resourceful
• Proficiency in technology (e.g.: Microsoft office and presentation software)
• Demonstrated ability to work independently and within a team
• Expertise and experience in cultural sensitivity and diversity
• Ability to speak French and other languages an asset
Travel Requirements:
Some travel in and around Algoma District to carry out the functions of this position, and to attend relevant meetings. Occasional travel outside of Algoma District may be required.
Valid driver’s license and access to a dependable vehicle are required.
Physical Demands:
• Performance of general office duties
• significant periods sitting at the computer, but with the opportunity to move away from the work station
• Sensory attention is required for the majority of the work day (looking at computer screen, reading documents, etc.)
Contract period November 2024 – August 28, 2026 with the possibility of renewal
