Lambton Public Health and Lambton Elderly Outreach have launched an online referral network for Sarnia-Lambton residents who need access to home and community care supports.

The site ‘Sarnia-Lambton Community Support Services,’ launched through Caredove, aims to simplify health and social service referrals. Partners include the City of Sarnia, Alzheimer’s Society of Sarnia-Lambton, Family Counselling Centre, March of Dimes, Seniors Home Independence Network, Canadian Red Cross, County of Lambton (Long-term Care & EMS), Grand Bend Community Health Centre and Philips Lifeline.

Funding was provided by the Ontario Ministry of Long-Term Care through the Health and Well-Being Grant Program.
